Medical Records or Adoption Paperwork

Before your appointment, we ask that we receive your cat’s prior medical records or adoption paperwork. This helps our doctors provide the best possible care.

You can:

Let us know where your cat was previously treated, and we’ll request the records on your behalf, or send photos or scanned copies in advance for us to review.

New Client Paperwork

We ask all new clients to complete our new client paperwork online before their first visit. This helps us save time during your appointment and focus on your cat.

Pre-Appointment Questionnaire

Before every visit, you’ll receive a pre-appointment questionnaire. This is your opportunity to share concerns, changes you’ve noticed, or anything you’d like the doctor to know ahead of time. It helps us tailor the appointment to your cat’s needs.

Arrival & Check-In

Appointment Timing

Please plan to arrive on time for your appointment. If you are more than 10 minutes late, your appointment may need to be rescheduled.

Carrier Policy

For the comfort and safety of all our patients, cats must be in a secure carrier while in the waiting room.

What to Bring

Please bring the following items with you:

Any medications your cat is currently taking
In original packaging or clear photos of the labels

A stool sample
Less than 24 hours old
About the size of a small Tootsie Roll

Copies of your cat’s medical records
Even if they were previously sent to us

Deposit & Payment Policy

A deposit is required at the time of booking a new-patient appointment. Deposits are refundable or transferable with at least 24 hours’ notice of cancellation. Some procedures may require an additional deposit at the time of booking.
